ÖZET
Amaç: Obezite, insülin sinyal mekanizmalarındaki bozulmalar neticesinde glukoz homeostazının kaybına ve sistemik metabolik disfonksiyona yol açan küresel bir sağlık sorunudur. Bu sistematik derlemenin amacı, obezite modellerinde uygulanan farklı kronik egzersiz müdahalelerinin; temel glukoz metabolizması belirteçleri üzerindeki etkilerini son 5 yılın verileri ışığında sentezlemek ve bu müdahalelerin göreceli etkinliğini değerlendirmektir.
Yöntem: Çalışma, PRISMA 2020 kılavuzlarına uygun olarak yürütülmüştür. Ocak 2020 ile Aralık 2024 tarihleri arasında PubMed, Scopus ve Web of Science veritabanlarında kapsamlı bir literatür taraması gerçekleştirilmiştir. Dahil etme kriterlerini karşılayan; obezite modellerinde en az 4 hafta süren kronik egzersiz protokollerini içeren ve spesifik glukoz metabolizması belirteçlerini nicel olarak raporlayan 11 çalışma analiz kapsamına alınmıştır. Seçilen çalışmaların metodolojik kalitesi SYRCLE ölçeği ile değerlendirilmiş ve bulgular nitel sentez yöntemiyle karşılaştırmalı olarak analiz edilmiştir.
Bulgular: İncelenen kanıtlar, tüm egzersiz modalitelerinin obeziteye bağlı glisemik bozulmaları iyileştirmede istatistiksel olarak anlamlı etkiler gösterdiğini doğrulamıştır. Yüksek Yoğunluklu İnterval Antrenmanların, özellikle insülin direnci indeksi ve dinamik glukoz toleransı üzerinde orta şiddetli sürekli egzersizlere oranla daha kısa sürede belirgin bir üstünlük sağladığı saptanmıştır. Kronik aerobik egzersizlerin açlık kan glukozunun uzun dönemli stabilizasyonunda; direnç egzersizlerinin ise kas kütlesi aracılı glukoz depolama kapasitesini artırarak postprandiyal glisemi yönetiminde efektif olduğu belirlenmiştir.
Sonuç: Kronik egzersiz müdahaleleri, obezite modellerinde glukoz metabolizması belirteçlerini normalize eden en etkili non-farmakolojik stratejilerdir. Maksimal glisemik kontrol için HIIT’in metabolik uyaran gücü ile aerobik ve direnç egzersizlerinin sistemik adaptasyon etkilerini birleştiren multidisipliner protokollerin en yüksek tedavi edici potansiyele sahip olduğu sonucuna varılmıştır.

The Effects of Different Exercise Interventions on Glucose Metabolism Markers in Obese Models: A Systematic Review of the Past 5 Years
ABSTRACT
Objective: Obesity is a global health problem that leads to impaired glucose homeostasis and systemic metabolic dysfunction as a result of disruptions in insulin signaling mechanisms. The aim of this systematic review is to synthesize the effects of various chronic exercise interventions applied in obesity models on key glucose metabolism markers based on data from the past 5 years and to evaluate the relative efficacy of these interventions.
Method: The study was conducted in accordance with the PRISMA 2020 guidelines. A comprehensive literature search was conducted in the PubMed, Scopus, and Web of Science databases between January 2020 and December 2024. Eleven studies that met the inclusion criteria—involving chronic exercise protocols lasting at least 4 weeks in obesity models and quantitatively reporting specific glucose metabolism markers—were included in the analysis. The methodological quality of the selected studies was assessed using the SYRCLE scale, and the findings were analyzed comparatively using a qualitative synthesis method.
Findings: The reviewed evidence confirmed that all exercise methods demonstrated statistically significant effects in improving obesity-related glycemic disorders. It was found that High-Intensity Interval Training (HIIT) provides a distinct advantage over moderate-intensity continuous exercise in a shorter timeframe, particularly regarding the insulin resistance index and dynamic glucose tolerance. It was determined that chronic aerobic exercise is effective in the long-term stabilization of fasting blood glucose, while resistance exercise is effective in managing postprandial glycemia by increasing glucose storage capacity through muscle mass.
Conclusion: Chronic exercise interventions are the most effective non-pharmacological strategies for normalizing glucose metabolism markers in obesity models. It has been concluded that multidisciplinary protocols combining the metabolic stimulatory power of HIIT with the systemic adaptive effects of aerobic and resistance exercises possess the highest therapeutic potential for achieving optimal glycemic control.
